Health & Wellness Specialist POSITION
Summary:
Supports and advocates for patients and their families pursuit of health and wellbeing.
Helps provide navigation through various health and social services to obtain resources that support the adoption of a healthy lifestyle.
Works closely with the primary care team, the Wellness Center, and resource providers to promote and improve the health of our patients.
Provides health coaching, education, and information counseling with a focus on self-management and health promotion.
Extensive community outreach, such as home visiting, is required.
ESSENTIAL JOB FUNCTIONS:
Responsible for establishing trusting relationships with patients and their families and various community partner organizations.
Utilizes motivational interviewing techniques to provide individual support and coaching for patients and their families.
Coordinates and manages closed-loop referrals to other community organizations.
Leads, teaches, and facilitates various health classes at the Wellness Center and other community partner sites.
Provides transportation to necessary health and social services to qualified beneficiaries.
Provides current and appropriate general health and wellness education throughout interaction with patients.
Identifies barriers and implements strategies to overcome barriers in efforts to connect patients to health and social services.
Works cooperatively with other clinical personnel assigned to same patient.
Operates Sky Lakes owned motor vehicles in a safe and professional manner.
Must be able to remain calm under stressful situations.
Frequent sitting, standing, and/or driving throughout the shift required.
Must be able to perform varied activities.
MARGINAL JOB FUNCTIONS:

Job Requirements
Qualifications:
REQUIRED:
Excellent driving record and ability to be insured by the hospital's auto insurance policy; Ability to work independently without constant supervision; Ability to effectively communicate with patients, medical personnel, community partners and others they may come into contact with, in a pleasant and constructive manner; Ability to prioritize workload in a timely manner; Ability to take direction from lead, manager or director regarding work activities and procedures.
PREFERRED:
Health/social service experience; experience working in multi-cultural settings, bilingual.
NON-REG CERT:
AHA BLS certification required by end of month from 60 days of hire and maintained; Possession of valid Oregon driver's license.
TYPICAL PHYSICAL/MENTAL DEMANDS:
Able to work as assigned every week, have active automobile insurance, driver's license and ability to drive safely throughout the geographic area defined by Sky Lakes Medical Center, in all types of weather.
Must be able to maneuver through unpredictable and unalterable physical spaces and to be able to climb minimum of two flights of stairs.
Must be able to work with periods of indirect supervision in patient's place of residence.
PAY RANGE:
$17.
35 - $25.

Estimated Salary: $20 to $28 per hour based on qualifications.
