3K a year 11 days ago 11 days ago 11 days ago Hybrid remote work in Klamath Falls, OR 97601 This position is responsible for the planning, organization, and execution of Health Equity outreach activities, including building and maintaining positive working relationships with participating community partners, as well as fulfilling regulatory requirements.
This position will operate as the facilitator of the local Community Information Exchange (CIE) website, ensuring that offered resources are properly claimed, updated, and responded to by the corresponding community partner.
This position will also serve as the primary coordinator for the Community Advisory Council (CAC), organizing and corresponding with internal and external partners to facilitate meetings and activities by performing the following duties.
Works alongside the community and partner organizations in delivering culturally responsive resources to members and the community by planning, organizing, and attending outreach activities.
Serves as the Community Advisory Council (CAC) Coordinator and the Systems of Care (SOC) Coordinator, which includes scheduling meetings, preparing agendas, minutes, and meeting packets.
Facilitates communication between the CAC, SOC, and the leadership team of the organization, ensuring the voice of the member is heard.
Conducts outreach to community partners and provides them with technical assistance to increase the number of claimed and utilized programs on the CIE.
Collaborates with internal teams to coordinate outreach efforts and ensure alignment with organizational goals.
Maintains accurate records of outreach activities, including participant information and outcomes.
Assists with administrative tasks related to outreach activities, such as scheduling meetings and preparing meeting materials.
Plans and organizes regularly scheduled and special outreach events, ensuring proper assignment of resources, identifying activity effectiveness and potential areas of improvement.
Assists with development of the CIE and community and member outreach to increase use of the CIE.
Schedules, coordinates, and records CAC and SOC activities.
Coordinates the administration of the Non-Emergency Medical Transport (NEMT) program, including data collection, analysis, monitoring, and auditing to ensure members are being safely and effectively transported to and from health resources.
Assists with the development of policies and procedures, workflows, flyers, and training materials.
Analyzes and reports on Key Performance Indicators (KPIs) against established performance expectations on all programs.
Facilitates program and project teams for implementation of health equity, social determinants of health, and member experience programs and initiatives as assigned.
Completes special projects or events which may require researching, reviewing and analyzing data, identifying best practices, recommending solutions, and writing reports, action plans, and program or project plans.
Attends state and community meetings and events as organization representative as assigned.
Maintains confidentiality and complies with HIPAA rules and regulations.
Maintains punctual, regular, and predictable attendance.
Works collaboratively in a team environment with a spirit of cooperation.
Displays excellent communication skills including presentation, persuasion, and negotiation skills required in working with members and coworkers and including the ability to communicate effectively and remain calm and courteous under pressure.

EDUCATION and/or EXPERIENCE Associate's degree (A.
A.
) or equivalent from two-year college or technical school; and two to four years related experience and/or training; or equivalent combination of education and experience.
COMPUTER SKILLS Job requires specialized computer skills.
Must be adept at using various applications including database, spreadsheet, report writing, project management, graphics, word processing, presentation creation/editing, communicate by e-mail and use scheduling software.
CERTIFICATES, LICENSES, REGISTRATIONS This position requires OHA certification as a Traditional Health Worker or the ability to obtain within six months of employment.
This position also requires a valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
TRAVEL REQUIREMENTS This position requires regular day trips in state for attendance at community outreach, events, and state meetings.
Bi-lingual in English/Spanish is preferred.
