Position is located in the __ Social Work Service at the Southern Oregon Rehabilitation Center & Clinics Klamath Falls Oregon Outpatient Clinic. The position is aligned to the Patient Aligned Care Teams (PACT) and is responsible for assessment, planning and provision of case management services in the CBOC's and Primary Care Teams at the Klamath Falls Community Outpatient Clinic. The incumbent coordinates a variety of activities including outreach, interdisciplinary activities, psychosocial assessments, treatment planning, consultation/education to medical center staff and community based service providers, the collection and maintenance of statistical data, program planning evaluation and modification, and the supervisor of student interns may be assigned.
Basic Requirements: Masters Degree: social work. Must fulfill all of the requirements for the master's degree in social work in a school accredited by the Council on Social Work Education.
License Requirement: Be licensed or certified at the Master's Level to independently practice social work in a state.
_For grade GS-12 level Senior Social Worker/Program Coordinators_
Experience, Education and Licensure. Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgment. At least two years should be in an area of specialized social work practice. Senior social workers must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice). Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
